Your membership includes:

5 visits in addition to the annual visit per year, 6 visits total annually ($75 dollars for each additional visit)
24/7 access to your NP who knows you!
Consultation and advice by phone for review of labs and non-urgent concerns
Email access for non-urgent concerns or questions
Comprehensive annual exam
Annual labs including CBC, CMP, Cholesterol panel, Thyroid, Glucose, and Urinalysis (more labs available based off of specific patient needs)
Medication refills
Vaccine prescriptions
Same-day or next-day appointments
Referrals to specialists as needed
Completion of forms, pre-authorization

Concierge medicine membership:

Each concierge medicine membership is a 12 month commitment and your credit card will be charged based upon the plan chosen. You may withdraw at any time with 60 day advanced notice. You will be refunded the prorated fee minus $400 if you have already had your annual physical examination For patients who are paying monthly, if you decide to withdrawal from the practice and you have already had your annual physical examination you will still be responsible for the $400 physical examination fee.
